Pure Simplicity

Simple tutorial:

black and white radial blur from the bottom upwards, filter>liquify (just do a random squiggle.)
new layer
apply image
then filter>artistic>plastic wrap,
new layer, apply image
filter>stylize>glowingedges,
new layer
apply image
then filter>noise>despeckle.
add text.
layer>new adjustment layer> color balance. play with the midtones and highlights, if you want you can do the shadows....
new layer
apply image
blending options, stroke 1pixel (do a color that goes with your picture.) this will create a border if you do it inside.

and your done. :)

the text i used is called :WST_Czec
i added a 2 pixel (100% hardness) line to the left hand side and a 2 pixel (100% hardness) to the top of the other bit
